MEDICAL coverage is provided by United Healthcare.
Employees have the option to enroll in Single or Family coverage. When you enroll in medical coverage, you pay a fraction of the total group premium, with Elevate paying a significant portion of the total premium.
DENTAL coverage is provided by Humana.
This employee-paid benefit offers both in-network and out-of-network coverage. Additionally, you may select a plan with or without orthodontic coverage.
VISION coverage is provided by Humana.

401(K) PLAN managed by TransAmerica
Elevate’s 401(K) plan gives employees an easy way to save for retirement, while reducing taxable income today. Elevate’s 401(K) plan includes an employer match.
PROFIT SHARING PLAN managed by TransAmerica
This employer-funded defined contribution plan allows for tax deferred accumulation of funds in the employee’s retirement account.
